CSV Extraction in Customers Page

Fixes

Player Username is no longer shown to affiliates due to security reasons.
When extracting CSV in summary form or with Statistic toggle, previously the CSV extract would always pull statistics and summary regardless if the statistic toggle was enabled or not. This is has now been fixed and will extract only the summary or summary and statistics depending on the choice.
A number of CSV extractions have been improved, in both their structure and cleanliness.
